Job Summary

A company is looking for a Nurse Practitioner (Remote) specializing in Complex Care.

Key Responsibilities

• Deliver exceptional care to complex patients using video telehealth visits and an interdisciplinary team
• Guide and oversee the medical management of patients utilizing the 5M framework
• Perform comprehensive assessments and coordinate care with the patient's support systems

Required Qualifications

• Master's or Doctoral degree in Nursing with a specialization in primary care, internal medicine, or geriatric medicine preferred
• Active RN and APRN license in AZ, CO, or TX required at hire
• Willingness to obtain and maintain multiple state licenses
• Three or more years of experience as an APRN in relevant fields
• Proficiency in telehealth technology and electronic health records (EHR)
