A company is looking for a Care Management Support Coordinator II.
Key Responsibilities
- Perform outreach to members to support care plans and connect them with resources
- Serve as front-line support for member and provider inquiries, explaining care plan procedures
- Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
- High School diploma or GED required
- 1 - 2 years of related experience required
- Bachelor's degree preferred
- Strong background in Case Management
- Knowledge of health plan activities and community resources is beneficial
