Job Summary

A company is looking for a Remote Licensed Practical Nurse with a CA License required.

Key Responsibilities

• Support healthcare delivery by performing activities under the direction of Nurse Practitioners and clinical policies
• Provide coaching and support to patients managing chronic conditions
• Conduct comprehensive health assessments and monitor patient progress through regular check-ins

Required Qualifications

• Active multi-state compact Practical Nurse License and CA state LPN/LVN license
• 5+ years of clinical experience as a Licensed Practical Nurse
• Experience in patient education for chronic condition management
• Highly skilled in behavioral-based coaching
• Prior experience supporting patients remotely is a plus
